The All on 4 procedure is used to replace the entire teeth of the upper jaw or the lower jaw. Typically the dentition is deemed hopeless; either damaged by periodontal disease or caries or heavy occlusion. There are alternatives to the All on 4 procedure, but this treatment can be done fast and without lengthy bone grafting procedures.
All On 4 can be done from start of the surgery to completion in 3 days. This is the reason why this procedure is so popular, because patients do not want to be without teeth for too long. As a general rule, the All On 4 avoids the need for bone grafting as there is usually plenty of bone at the front of the jaw, where we avoid the sinus and lower jaw nerves.
Once the implants have been placed in the jaw bone, an impression is taken and sent to the laboratory and the denture is connected to the implant on day 3. The gums and bone are still healing, and at 6-8 weeks final adjustments are made to the denture.
In the lower jaw, the bone is much thicker and can support the entire denture on four implants. In the upper jaw, the bone is thinner, and yes four implants can support the denture but there have been known cases of overload failure due to heavy biting. In the upper jaw it is much safer to place five or six implants to distribute the load.

The dental brace or orthodontic bracket is principally used in the correction of misaligned jaws, crooked teeth and jagged or irregular bites such as underbites and overbites. Braces come in many varieties. There are the regular metallic braces, the clear braces, and lingual braces. All three of them will correct severe, moderate and mild crowding. Before braces are placed into the mouth, the dentition needs to be healthy first. Once the dentition is in good health, and oral hygiene is good, the process of applying braces can commence.
The first stage of braces is the measuring process. This involves taking of several X-rays, the OPG, Lateral Ceph, and sometimes the hand wrist X-ray. Impressions are taken of the mouth and many measurements are recorded. Once all these measurements are done, a treatment plan is devised to correct the misaligned teeth jaws. Each case is different and requires careful planning.
In young children, growth modifying appliances are sometimes used first before braces are put in to correct bone related issues. In adults, growth has completed and we cannot modify the growth of the jaws as much.
Treatment times for braces can vary from 6 months to 3 years.
The clear ceramic braces are tooth coloured, but have a tendency to break off. As a result of them breaking off, there are more visits to place new brackets in, which can extend the treatment time and cost.
The alternative to braces is invisalign. Invisalign is can correct only mild cases of crowding and can only tip teeth, whereas braces have the full range of tooth movement allowing it to correct severe cases.

A bridge is one alternative to replace a missing tooth. It is basically two or more crowns joined together to form a “bridge” across the missing tooth.
Treatment Procedure – Dental Bridges:
The procedure requires two appointments. During the first visit, we will examine the health of your gums and other teeth to evaluate if you are a candidate for a dental bridge. If you are suitable, you are given a local anaesthetic so your dentist can prepare the teeth required to support the bridge. If the support teeth are decayed or badly broken down, your dentist may have to build them back up before they can be used as support teeth for a bridge.
Next, your dentist takes an impression of the prepared teeth with a silicon-putty material that is used to create a model of your teeth. Your bridge is fabricated based on this model by a skilled lab technician so that it precisely fits the prepared teeth. It is important that your restoration fit perfectly to avoid additional oral health problems such as tooth decay. While your bridge is being fabricated, your dentist fits you with a temporary bridge so the teeth and gums can be protected from damage until your permanent bridge is ready. To complete the dental bridge procedure, you must return to the dental office for a second visit to have the bridge fitted and cemented.

If both adjacent teeth are healthy untouched teeth we recommend the dental implant instead. The average lifespan of the tooth is about 10-20 years under normal biting pressures. There are several type of bridges, including conventional, resin bonded, porcelain fused to metal, all metal, and cantilever bridges. The advantage of invisalign is that it is a clear teeth straightening system, and you can take them off if you if you need to attend an important function. The invisalign aligners are to be worn 23 hours a day and for 2 weeks at a time. The only time you take them out is when you eat. Every two weeks a new aligner is fitted and gradually your teeth are moved into perfect alignment.
The disadvantage of invisalign is that it is more expensive than normal braces and it is reliant on patient compliance. Braces are stuck on your teeth and there is no choice about it, whereas with invisalign, some patients forget to wear the aligners, and when they come to see the dentist, they will say they have worn the aligners but they have not. This can affect the success of the invisalign as the aligners will not fit properly if they are not used correctly.
